To get a clear picture of your general health and dental wishes, please fill in this form as completely as possible. Upon receipt, we will send you an appointment by e-mail. This is final unless we receive a cancellation from you. Should we not be able to comply with your request, we will contact you by telephone. In case you have difficulty walking and climbing stairs, we advise you to report this when making the appointment. We will then schedule and perform your treatment in a treatment room on the ground floor. If you have pain complaints that need to be remedied immediately, you can also indicate this on the registration form. We will of course take this into account when planning your first appointment.
